Brief Description
As the Registered Nurse Case Manager, you will work independently with groups of clients to provide initial and on-going assessment of the client’s skilled needs. Implement individual plans of care and make referrals to others disciplines and/or revise the plan as necessary. Provide ongoing consultation and education for patient/family member regarding all aspects of care. Communicate and collaborate with family /caregivers and other professionals involved in the care of the client. Supervise Licensed Practical Nurses and Home Health & Hospice Aides.

Full-Time, 8:30 am to 4:30 pm (Monday through Friday) (Leadership; On-call Rotation)

Westminster Canterbury Richmond at Home provides skilled nursing and therapy services to our home health & hospice residents in Independent and Assisted living as well as to the Greater Richmond community at-large who are experiencing complex deviations in health. Through clinically grounded care plans and a holistic approach towards physical restoration and healing, Westminster Canterbury at Home’s care team restores our residents health, maximizes their functional capacity and helps them maintain a healthy and vital lifestyle while aging in-place.

Requirements
MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:
- Complete initial and ongoing comprehensive assessments of the client’s needs, including initiating the plan of care and revising as necessary
- Provide skilled nursing care services and/or treatments requiring substantial and specialized nursing skill, including intravenous fluids and medications, urinary catheterization, and complex wound
- Counsel the client and family in meeting nursing and related needs
- Initiate appropriate preventive and rehabilitative nursing procedures
- Prepare clinical and progress notes
- Coordinate services involved in client’s care
- Make referrals to other services as needed
- Inform the physician and other staff of changes in the client’s needs
- Evaluate outcomes of care and complete the OASIS assessment accurately
- Supervise LPN/LVNs and Home Health & Hospice Aides providing services
- Assign home health & hospice aides to a specific patient
- Participate in in-service programs
- Plan for the discharge from service
- Comply with agency policies and procedures relative to infection control and safety
- Must be able to perform cardiopulmonary resuscitation and relief of an obstructed airway; climb at least 2 flights of stairs and be able to stand and/or walk for a period of at least 4 hours
- Current and unrestricted license as a Registered Nurse in the Commonwealth of Virginia
- Current CPR certification for healthcare provider
- Basic computer skills
- Must possess good verbal and written communication skills
- Valid Driver’s License in the State of Virginia with a minimum age 21 to drive for the organization
- Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers
- Proof of motor vehicle insurance
- Graduate of an accredited school of professional nursing
- Two years of nursing experience, preferably in Hospice or Home Health
- High standard of Displays actions consistent with statements. Truthful; elicits trust and respect. Approachable and open to constructive feedback.
- Strong sense of teamwork and cooperation; inspires team to meet challenges & implement Works cooperatively with others; success in building alliances within and among work units to achieve goals and objectives.

- Sound judgment to make decisions promptly based on analysis of situations when dealing with challenges and/or with adverse conditions and to communicate them with clarity, purpose and commitment.
